And I would do the sizing that Gary described with the cradle doo-dad in the boot.  That makes the boot narrower and a better heel fit, and it's a lot easier to remove material from that cradle that to add to the boot, but you don't want to buy, find it's loose, and then try to decide if the cradle can fill enough space.
